NHS Future Fit

People from Telford & Wrekin and Shropshire have been having their say on the future of their healthcare services throughout February and March.

The NHS Future Fit team organised a series of pop up stands/shops which will give local people the opportunity to play their part in helping to shape how healthcare is delivered for the next 20 years.

The first of the pop up shops kicked off in February at Telford Shopping Centre.

Dr Caron Morton and David Evans, Joint Senior Responsible Officers, said: “This is a great opportunity for people to take a break from their shopping and have their say on a really important issue – the future of NHS healthcare services locally.

Patient Online Webinars

You may already be aware of the initiative called Patient Online which aims for 95% of GP practices to allow patients online access to summary medical records; appointments and prescriptions by 31 March 2015.  

There are a series of webinars that NHS England is offering to help GP practices better understand the initiative and provide an opportunity to ask questions.

Dignity Day Success

A recent roadtrip around Telford and Wrekin’s nursing homes called on green-fingered and crafty residents to 'put the Dig and Knit' back into dignity.
Members of the CCG Safeguarding and Quality teams, together with colleagues from Shropshire Community Health Trust, visited all 53 of the area’s nursing, residential and learning disability homes, for 2015’s Dignity Action Day in February.

Christine Morris, Executive Nurse at the CCG, said: “We wanted to do something creative this year with Dignity Action Day, and while nationally the Dignity in Care campaign will be putting on Digni-Tea mornings across the country, we wanted to place the emphasis on something that allowed our residents to get involved.

“So, this year we took a packet of sunflower seeds to each of the homes to plant, and asked staff and residents to record the heights and send us photos of the resulting blooms as they come to flower,” she said. “At the same time, we invited all of the homes to knit a 15cm by 15cm square, which we collected and sewed together to produce a Telford and Wrekin Dignity Blanket.

“We wanted to give that finished blanket to someone who is less fortunate than us and will be donating it to the Telford Community Store.”


Safeguarding: Prader Willi Syndrome Update

In 2014 Staffordshire and Stoke on Trent Adult Safeguarding Partnership Board commissioned a multi-agency learning review following the death of a young adult who was diagnosed with Prader-Willi Syndrome.

This briefing note has been prepared to raise awareness of this rare and complex condition.

Professional Education, Development and Support for Telford and Shropshire GPs
We are committed to supporting the education and development of GPs in Telford and across the county of Shropshire.

On the Paean web site you will find details of all educational opportunities for GPs across the county and some outside Shropshire. We have details of Protected Learning Time Events in Telford and Wrekin as well as presentations from previous events. The Paean resources page includes ideas for your in-house events as a guide to using these half-days effectively.

Stop Smoking and Stop Smoking in Pregnancy Services

Public Health Telford and Wrekin recently undertook a tender process for the provision of Stop Smoking and Smoking in Pregnancy Services across Telford and Wrekin. 

As part of this tender process Telford & Wrekin Council gave partners which included GP’s and pharmacies the opportunity to comment on the delivery model. All information received was considered when developing the new service specifications.

The current Stop Smoking provision will continue until 31st March 2015 with existing providers.To read more about the new contracts click here.

Care Act Leaflets

Below are two leaflets about the Care Act which is being implemented in April 2015. takes members of the public and medical staff to local pages all about the Care Act and what is happening in the Telford and Wrekin area. It also has links to the national material being distributed.

Integrated Chronic Lung Disease Service

This service is commissioned by the CCG via the Community Trust and supported by a Consultant Chest Physician from SaTH. 

It includes community respiratory nurses, physiotherapists, healthcare assistants and Dr Nawaid Ahmad (Consultant Chest Physician) from SaTH.  There is a fortnightly community chronic lung disease MDT as well as a community clinic at Woodside Medical Practice. The MDT and clinic alternate every week on a Tuesday. 

At the moment the MDT focuses on patients with COPD and Bronchiectasis. Asthma and ILD patients are not currently included in the MDT discussions. 

The clinic has a set referral criteria which is available on the CCG website. It is a "one stop" clinic at the moment, where patients get seen, diagnosis is confirmed, disease education is imparted and treatment rationalised. There is a plan to expand the role of this clinic in the future. 

We would like to raise awareness of this service amongst our primary care colleagues so that we can strive to make chronic lung disease care in the Telford and Wrekin area a priority.

Prostate Cancer Awareness Month

A message on behalf of Dr Northern

"Just a short note to let you know that next month is Prostate Cancer Awareness Month. This is an annual event supported by cancer charities including Prostate Cancer UK. This year it is being promoted by Macmillan Cancer Support within the West Midlands region. Information about prostate cancer will be appearing in the media, encouraging men, particularly over the age of 65 years with the symptoms of prostate cancer to seek advice from primary care. I would like thank you in advance for  your support in this campaign during a busy time of the year. I have also contacted our Urological colleagues at SaTH so that they are aware."

Stop Press: Latest on the ICG Referrals System

The last quarter saw the latest enhancement of the ICG System rolled out across all Practices. The Practice Dashboard now enables all users to be able to drill down into a wealth of information and compare referral volumes at their Practice by GP, Specialty, Priority and Provider outcomes, all the way back to June 2012.

The next version of the ICG system is nearing the end of development and is due to be launched shortly. This latest version promises to be even easier to use than the current system and we will let you know more on this in the next issue.

In the meantime, TRAQS would like to extend its thanks to all Practices for their part in the roll out of the ICG System.
