Bluegrass Community Health Center begins to offer primary care based Hepatitis C treatment

Providers at the Bluegrass Community Health Center (BCHC) in Lexington, Kentucky, attended Kentucky Hepatitis Academic Mentorship Program’s (KHAMP) inaugural trainings in August, 2018 and February 2019. Since initiating this collaboration, the clinic has begun to offer Hepatitis C treatment at the clinic.

Screening of patients in the clinic has uncovered 227 cases of active patients with hepatitis C, of which 50% have been referred to specialty care due to advanced fibrosis scores, co-infection with hepatitis B, or patient preference. The remaining patients with active Hepatitis C have initiated workup needed for treatment at the clinic and 8% have completed the entire process required to initiate treatment at BCHC. This involves a virtual referral to faculty at the University of Louisville Hepatitis C Center who helped develop the KHAMP program.  Six months into initiation of the program, our first 5 patients are currently undergoing treatment and we expect an additional 20 to initiate treatment in the next month.

The implementation of this program has required collaboration between providers, nursing, behavioral health, care coordinators, and specialty pharmacy services. The specialty pharmacy services provide prior authorization and case management support.

This service is being provided in conjunction with expansion of the clinic’s medication assisted treatment (MAT) program, which provides buprenorphine-naloxone and naltrexone based therapies along with  integrated behavioral health services.
