August 2015 CareConnect Update
Welcome to our overview of what is happening in the CareConnect programme

ARI Programme

The ARI programme provides early intervention for 'At Risk' patients as well as re-establish general practice as the centre of coordinated care in CM Health. The patient has an assigned care coordinator, a CarePlan, and an “eSummary health record” created, reviewed and made available to everybody involved.
To facilitate this programme CareConnect Shared Care team worked closely with both the DHB implementation team and the vendor to ensure that the supporting IT tool was fit for purpose, including new functionality and reporting.
We then provided full training and resources to PHO support teams to ensure that all GPs are supported in the roll out of ARI.



Dynamic Pathways enable clinicians to follow evidence based investigation and treatment plans from within their practice PMS.
The Proof of Concept for Dynamic Clinical Pathways, using the Nexxt pathway tool, kicked off on Monday 11th August.
The Pathways that we are using for the POC are:
  • Dyspepsia
  • GOUT
  • COPD
  • Cognitive Impairment
  • Iron Deficiency
The POC runs for 3 months and then a full pilot will be established
The Practices that have agreed to participate in our Proof of Concept are:
  • Eastside Family Doctors
  • Pakuranga Medical Centre
  • Avondale Medical Centre
  • Whangaparaoa Medical Centre
  • Milford Family Medical Centre
  • Bush Rd Medical Centre
  • Hokianga Health
  • Marsden Medical Practice

Stats as of 28 August:

20   Number of medical professionals using a pathway

80  Number of patients that have been put on a pathway

158 Number of times a pathways has been accessed

Exciting Times in Northland....

2 initiatives are now underway in Northland for 3 month trial periods.
  1. Secure messaging for Primary and Secondary care communications
  2. Two GP practices are participating in the Dynamic Nexxt Pathways proof of concept (see above)
Bush Rd and Hokianga Health are participating in both of these initiatives.  Secure Messaging via the Shared Care tool will enable hospital clinicians and GPs to communicate with each other directly in a secure and patient centric manner. 
All clinicians at Northland DHB will be able to see a summary patient record via Concerto of the patients registered for Shared Care at those 2 practices.
SMO's from the following Hospital Services are participating in the Secure Messaging pilot:
  • Diabetes
  • Renal
  • Paediatrics
  • Cardiology
Sophie and Maureen (pictured) at Hokianga Health will be enrolling patients and supporting their GP's with both of the initiatives, Secure messaging and Nexxt Pathways.
