Chailey Heritage Pathways
Chailey Heritage Pathways will offer highly specialist care for children, young people and young adults with physical disabilities and/or sensory impairments and associated health needs both at home or in the community. Chailey Heritage Foundation is recognised as one of the UK’s leading centres for children and young adults with neurological motor impairment such as Cerebral Palsy. Chailey Heritage Pathways channels all of that experience, expertise and commitment into the wider community.
How We Can Help
We will provide community support whenever and wherever needed. This could range from a few hours to all day, at weekends, evenings, before or after school and during school holidays. Our team of highly skilled support workers will work closely with individuals, family members and other professionals to deliver a person centred package of care and support. Each team member will have all the necessary training to ensure each individual's needs can be met, this will include:
• Enteral feeding e.g. PEG/JEJ
• Epilepsy & emergency medication
We will also look to develop skill sets in order to meet further or changing needs.
