Ontario Health Teams: Providing Connected Care for Francophones

To assist future Ontario Health Teams as they work to complete the full application to submit to the Ministry of Health, Entité 4 has developed Fact Sheets on providing Connected Care for Francophones.
The Fact Sheets contain information on the Francophone community and available French Language Services within various LHIN Sub-Regions.
Data for LHIN Sub-Regions is recognized by the Ministry of Health as an acceptable proxy for specific minority populations within the population served by groups working to submit the full application to become an Ontario Health Team.
